Problem

Care home staff need one system to manage patient care plans, monitor vitals, and track staff performance instead of juggling separate tools and paper logs across professionals, patients, and management.

Solution

Built a role-based care platform with three roles: care professionals manage their patient list, build and edit care plans, monitor vitals live from connected devices, get real-time alerts on abnormal readings, and can chat with an AI agent for support; managers oversee staff logs, performance metrics like hours worked, and delayed-care tracking across the clinic; patients get their own view of their info. Care plans and medication charts can be downloaded directly from the dashboard.
